Client's Portal View: Documents & Forms

This tutorial will assist clients in accessing, viewing, signing and sharing documents and forms within the client portal.





When logging into the client portal, the client's view toolbar starts with the Documents & Forms tab on the left.

The first section within Documents & Forms is for the client's Consent Forms. In this area, any consents shared by the practitioner, will be available for the client's signature acknowledgement.

Intake Forms:

Although the header of this section says Forms, it includes forms shared as Intake, such as the Credit Card Authorization form, Release of Information, Intake Interview, etc.

Note: The Forms in this section will also include any Assessments which were cloned as Intake Forms.



Shared Documents:

The shared documents section will include Assessments that are shared with clients. The labels on the Assessments will indicate their status for the client.

  • Draft: Indicates the Assessment is pending completion by the client.
  • Current: Indicates a completed and active Assessment.
  • Past: References a completed and stored Assessment and there is a current one in the client's chart.

Note: An exclamation point on any Assessment indicates that it has been activated on the practitioner's side and is pending the client's signature acknowledgement.



My Uploads:

The uploads section is an area for the client to share documents or uploads with the practitioner. This section is useful to upload homework assignments, or share photos or artwork. The client has the flexibility to edit or remove any shared uploads by clicking the "..." menu. The practitioner may access the uploaded files from the client's Files section of their chart under Uploaded by the Client.


Click Add to upload an attachment.

Almost any type of attachment is supported.
