Understanding Signature Requirements for Different Form Types

When sending forms to clients, it’s important to know which forms include embedded electronic signatures and which require manual configuration. Below is a breakdown by form type:


Consent Forms

  • Embedded Signature Included: These forms automatically include an electronic signature field.
  • Client Signature Prompted: Clients will be prompted to sign when the form is sent—no need to manually add a signature field.
  • Provider Countersignature (Optional): If a provider signature is needed, enable the countersignature option when creating the form.

Assessments

  • Embedded Signature Included: These forms come with an electronic signature field.
  • Client Signature Prompted Automatically: No need to add a separate signature element.
  • Built-in Countersignature: When sent via the client’s Assessments tab, the system allows you to complete and sign the form after the client submits it.

Intake Forms

  • No Embedded Signature: Signature fields must be configured manually.
  • Manual Signature Setup: You must add an electronic signature element and enable the countersignature option during form creation.

Questionnaires

  • No Embedded Signature: These forms require manual setup.
  • Manual Signature Element Needed: You must add an electronic signature element when creating the form.
  • No Countersignature Option: Questionnaires do not support provider countersignatures.

Notes

  • Automatically Signed Upon Completion: Notes are created for each session and do not allow adding an electronic signature element manually.
  • No Countersignature Needed: Once a note is marked as complete, the system will prompt you to sign it automatically—no additional configuration is required.
