Client Intake Overview
This article walks through the client experience after a provider has invited you to the client portal. As a client, you may also view our Orientation to the client portal for clients video.
Once invited to the portal, you will receive an invitation email.

After clicking on Accept Invitation, you will arrive on the account creation screen.

Email - this is filled in with the registered email on your client record.
Password - Enter a password based upon the requirements.
Password confirmation - Confirm the password.
After creating an account, you will be presented with any Consent Forms assigned. At the bottom of the first Consent form, enter your electronic signature and agree by checking a box. Once entered, you will not need to enter the electronic signature on further forms, only check the box.

After signing all the Consent forms, you will see a Let's get started message. Clicking the button will advance the intake process.

Name
You will then be presented with a wizard that will collect any demographic information selected at the time of the invite. The first section collects name information.

First Name - Client first name
MI - Client middle initial
Last Name - Client last name
Pronouns - Desired pronouns to be used
Add Legal Name - If the you have a different legal name, you may enter it after clicking the Add Legal Name button.
Additional Demographics
In the Demographics section, fill out additional demographic information.

Birthday - Client's birthday
Sex - Client's sex
Gender Identity - Client's gender identity
Contact Information
The next step will ask you to fill out your contact information. The Time Zone selection will determine what time zone to use for appointment reminders.

Contact Preferences
The final demographics step will ask you to fill out communication preferences.

Intake Forms
After entering demographic information, you will be presented with any Intake forms selected. At the bottom of each form, you may click the Skip link if you do not want to fill out a particular form at that time. When the final Intake form has been filled and signed, the Intake is complete.

Note: Answers are autosaved and if a client returns to a form, previous responses will have been filled in.
Incomplete Intake Forms
When required questions are incomplete, they will be highlighted in red. For those who have difficulty seeing the color red, the Save button tooltip will contain a summary of incomplete questions.

Please Note: The Client Portal contains important health care records and documentation that are maintained by your therapist as part of your clinical care. Because these records are the responsibility of your therapist, Sessions Health is not authorized to delete or modify them on your behalf.
If you would like to remove your access to the Client Portal, please contact your therapist directly. Additionally, any questions or requests regarding your medical records or personal health data — including corrections, deletions, or transfers — should be discussed and handled through your therapist, as they are the custodian of your clinical information.
