Supervision: Notes and Treatment Plans
Supervision: Summary
Sessions Health’s enhanced clinical supervision tools streamline how supervisors guide and support their supervisees. Supervisors can easily review and co-sign progress notes and treatment plans, with clear alerts on the Home page whenever documentation is ready for review or signature.
Administrators can customize co-signature requirements for both progress notes and treatment plans, turning them on or off as needed, to simplify workflows and promote timely, collaborative supervision.
Permissions: A team member must be assigned the Supervisor role and linked to the practitioner they oversee in order to activate supervision features. For detailed steps on configuring supervisors, supervisees, and signature settings in your account, please refer to our Managing User and Roles guide.
Supervision: Progress notes
Once the supervision settings are configured, after a supervisee signs a progress note, the session note will display “Awaiting supervisor review” and a "Pending Supervisor Review" banner will appear in the appointment entry on the Summary page of the client's chart.
Both supervisors and supervisees can view and manage progress notes by opening the appointment from the Calendar view and clicking the Notes subtab.
The supervisor will then have an entry in their Home > Needs Attention > Needs Signature section.
From there, a supervisor can click the session note link to review the details, Sign & Approve, or Request Edits on the note.
Request Edits Process
When Request Edits is selected, a modal will appear allowing a supervisor to enter comments to notify the supervisee that changes are needed. Once feedback is finalized, proceed to click Submit Request.
Submitting the request automatically unlocks the supervisee’s session note for editing.
Next, a new notification will appear in the supervisee’s Home > Needs Attention > Needs Revision section, restarting the note-signing process. By clicking on the session link from the Needs Revision panel, the system will route to the client's session note, which will display a yellow Needs Edits status along with the content of the supervisor’s feedback.
After a supervisee has completed the requested edits, a new entry will appear on the supervisor’s Home > Needs Signature panel, where they may click the link to open the session note and select Sign & Approve > Sign & Complete, or request additional edits.
Note: The system will display the supervisor’s latest Request Edits comment.
Lastly, after both the supervisor’s and supervisee’s signatures are applied to a progress note, the note will display a timestamp for each signature.
Note: Any notes signed by a supervisee before a supervisor was assigned will not be associated with the new supervisor. To associate these notes with the newly assigned supervisor, the supervisee must first unlock and re-sign the note. Once updated, the note will appear in the supervisor’s Needs Attention area for review.
Note: The ability and timing of unlocking a note depends on the account’s Notes settings. Administrator permissions are required to access this setting. Please reach out to your practice’s point of contact for assistance.
Supervision: Treatment plans
Administrators of an account can set up supervisees to have their client treatment plans reviewed and signed by a supervisor. This ensures better oversight and streamlines the workflow between supervisee practitioners and supervisors.
How to enable supervision functions
To enable supervision features: A system administrator must first assign the Supervisor role and configure the related settings by following the instructions in the Managing Users and Roles guide. Once a supervisor is assigned, additional options will appear to enable supervision for both progress notes and treatment plans.
Treatment Plan Review Process
Completion by Supervisee: When a supervisee creates a treatment plan under the Diagnosis & Treatment tab, it is automatically assigned to their supervisor for review. They will see a “Pending Supervisor Review” banner.
Supervisor’s Home Dashboard: Supervisors can access this from the Home > Needs Attention > Needs Signature section.
Review Actions: When accessing a treatment plan, supervisors have three options:
- Unlock: Removes the supervisee signature, requiring the supervisee to sign the treatment plan again. Unlocking the treatment plan will create an entry in the supervisee's Home > Needs Attention > Needs Revision section.
- Request Edits: Supervisors can provide the supervisees feedback in real-time. Requesting edits will unlock the treatment plan and create create an entry in the supervisee's Home > Needs Attention > Needs Revision section.
- Sign & Activate: This option activates the treatment plan, marking it as complete.
Notifications and Workflow
Needs Revision Status: If a supervisor unlocks the treatment plan or requests edits in the comments pop-up window, supervisees will see an entry in their Home > Needs Attention > Needs Revision section. The supervisor's comments will appear at the top of the treatment plan that needs to be revised, along with the comment date.
Once the supervisee re-signs the treatment plan, it will appear again in the supervisor's Home > Needs Attention > Needs Signature section.
