Supervision: Notes
Supervision in Sessions Health helps supervisors review and co‑sign clinical notes to ensure documentation is accurate and complete. Once a supervisee signs a note, it routes to the supervisor’s dashboard for final sign‑off.

ℹ️ Before you begin: Supervision applies to all notes that require a co‑signature. Supervisees create and sign notes, supervisors review or request edits, and administrators configure supervision settings and permissions.
In this article:
Supervision Workflow for Notes
Notes follow a structured review process to ensure accuracy and compliance. Here’s the full workflow at a glance:
- The supervisee signs the note to start the review.
- The supervisor reviews it and either approves or requests edits.
- If edits are requested, the supervisee updates and re‑signs.
- The supervisor completes the final signature.
- Both signatures appear on the finalized note.
Step 1. Supervisee signs the note
After the supervisee signs:
- The note displays Awaiting supervisor review.
- A Pending Supervisor Review banner appears in the appointment entry.
- The note routes to Home > Needs Attention > Needs Signature on the supervisor’s dashboard.
- Supervisors and supervisees can open the appointment and select Notes to view or manage the note

Step 2. Supervisor reviews the note
From Needs Signature, the supervisor opens the note and chooses:
- Sign & Approve

Request Edits

Step 3. Supervisor requests edits (optional)
What happens when Request Edits is selected
- A modal appears for entering comments
- Submitting the request unlocks the note
What the supervisee sees next
- The note appears in Home > Needs Attention > Needs Revision
- A Needs Edits banner displays the supervisor’s comments

Step 4. Supervisee revises and re‑signs (optional)
The supervisee:
- Opens the note from Needs Revision.
- Reviews the supervisor’s comments.
- Makes updates.
- Re‑signs the note.
After re‑signing, the note returns to the supervisor’s Needs Signature panel.

Step 5. Supervisor completes the review
- The supervisor selects Sign & Complete to finalize the note
- If more edits are needed, the supervisor returns to Request Edits (Step 3)
After completion, the note displays timestamps for both signatures.
Notes signed before supervision was assigned
Notes signed before a supervisor was assigned do not automatically route to the new supervisor. The supervisee must unlock and re‑sign them.
ℹ️ Unlocking availability depends on the account’s Notes settings. Administrator permissions are required to access this setting.
Frequently Asked Questions
Do supervisors need the Practitioner role to review documentation?
No. Supervisors only need the Supervisor role. They are not billed unless they also have the Practitioner role.
Can supervisors edit notes directly?
No. Supervisors can unlock or request edits. However, supervisees make the actual changes.
Do supervisees receive notifications when edits are requested?
Yes. Notes appear in Home > Needs Attention > Needs Revision with supervisor comments.
Does client‑level supervision affect progress notes?
