Supervision Structure
Supervision in Sessions Health is built on three core elements that determine how documentation is routed and reviewed.
Supervisor role
A team member must have the Supervisor role to review or co‑sign documentation. This role allows supervisors to access supervisee notes and treatment plans.
Practitioner‑level supervision
Administrators can assign a default supervisor to a practitioner. This determines who reviews and signs that practitioner’s documentation unless a client‑level override is applied.
Client‑level supervision
Some practitioners work with different supervisors depending on the client. Client‑level supervision allows you to assign a supervisor for a specific client without changing the practitioner’s default supervisor. This override applies only to that client’s documentation.
ℹ️ If a client-level supervisor is assigned, they will override the practitioner-level supervisor for that specific client’s documentation.
Supervisor and Supervisee Responsibilities
Supervisor responsibilities
- Review documentation submitted by supervisees
- Request edits and provide feedback
- Unlock documentation when changes are needed
- Sign and complete progress notes and treatment plans
- Access items awaiting review from Home under Needs Attention
Supervisee experience
- Create and sign documentation
- Receive edit requests and view supervisor comments
- Update and re‑sign items after revisions
- Reassign supervisors for their own clients when appropriate
- See items needing updates in Home under Needs Attention
Assign a Client‑level Supervisor
Use this workflow when a practitioner needs a different supervisor for one client.
To assign a client‑level supervisor
- Open the client’s chart
- Select the three-dot menu in their chart header
- Choose Reassign Supervisor
- Select the supervisor who should review this client’s documentation
- Select Save

How client‑level supervision works
- New notes and treatment plans for this client route to the selected supervisor
- The practitioner’s default supervisor remains unchanged for all other clients
- You can revert to the default supervisor at any time
- Changes appear in the client’s audit trail
- Historical notes and completed signatures do not change
ℹ️ Supervisees can reassign supervisors for their own clients. Administrators with the Supervisor role can update supervision for any client in the practice.
Supervision Across Document Types
Supervision follows a consistent review and signature flow across supported document types.
Progress notes
- When a supervisee signs a note, it enters Awaiting supervisor review
- A Pending Supervisor Review banner appears in the appointment entry
- The note routes to the supervisor’s Needs Attention list
- Supervisors can sign or request edits
- If edits are requested, the note unlocks and returns to the supervisee
Treatment plans
- Completed plans route to the supervisor for review
- Supervisors can unlock, request edits, or Sign and Activate
- If edits are requested, the plan returns to the supervisee until updated and re‑signed

Can a practitioner have more than one supervisor?
Yes. A practitioner can have one default supervisor and different supervisors assigned to specific clients.
Does supervision change how documentation is routed?
Yes. When a supervisee signs a note or treatment plan, it routes to their assigned supervisor for review. Client‑level supervision overrides the practitioner’s default supervisor for that client only.
Can supervisors see supervisee drafts before they are signed?
No. Supervisors only see documentation after the supervisee signs it or submits it for review. Drafts remain visible only to the supervisee until they are ready for review.
Does supervision affect who can unlock documentation?
Yes. Supervisors can unlock the documentation they are responsible for reviewing. Administrators with the Supervisor role can unlock items for any supervisee in the practice.
